The NACPAC Pain Clinic Registration Form

We welcome submissions of Canadian Pain Clinics or Pain Specialists to add to NACPAC's online Directory of Canadian Pain Clinics and Pain Specialists. Applications from General Practitioners who treat patients with chronic pain are also welcome. Due to the national high populace and limited website maintenance, we are sorry that we cannot accomodate applications from Physiotherapists or Physiotherapy clinics, Chiropracters, Massage Therapists or related therapy based clinics, including any practitioners of Alternative Medicine.
